Apparently, you can also use the patient logs as aliens often bypass Reception and GP’s. Even watching out for these two things, it still took some time. In addition, they will usually bypass the reception area. I had the most luck by putting the game at slow speed and watching the area near the reception. They will behave oddly, but I found that hard to spot in amongst the regular patients (who also behave oddly). They look like normal patients for the most part.
